What the Planet Fitness cancellation form is

It is a paper form kept behind the front desk at the club, not a document Planet Fitness publishes for download. You ask for it at the home club listed on your membership agreement, fill it in, sign it, and hand it back. Clubs are mostly franchise owned, so the layout differs from one to the next while the information it collects does not.

If you cannot get to your home club, you are not stuck. A written cancellation letter mailed to that club is an accepted method, and it carries the same information the form does. That is what the generator further down produces.

What the form asks for

Bring these with you, or put every one of them in the letter. A missing membership number is the usual reason a cancellation sits unprocessed.

  • Your full name, exactly as it appears on the membership.
  • Your membership or key tag number, from your card, the app, or a billing statement.
  • The home club on your agreement.
  • Your mailing address, phone number, and email.
  • Today's date and the effective cancellation date you are requesting.
  • Your signature.
  • A reason for cancelling, on forms that ask. This is normally optional.

Add one line the form itself will not include: a request for written confirmation that the membership is cancelled and that future billing has stopped. Without it you have handed over a piece of paper and kept nothing.

Mailing it instead: the certified mail route

Print the generated notice, sign it, and mail it to the home club street address on your membership agreement. Do not send it to a corporate address; the club that holds your account is the one that has to act on it.

Use certified mail with return receipt. The receipt puts the delivery date in writing, and that date is what decides which billing month the cancellation lands in if a charge is ever disputed.

Timing decides whether you pay one more month. Clubs commonly need the request before a cutoff date in the billing month for the next draft to stop, and an annual fee can still fall due if it lands inside your notice period. The exact cutoff, notice period, and any remaining commitment term are in your membership agreement, so read that section before you choose the effective date.

One thing that never works as a substitute: cancelling the card. Dues at many clubs are drafted by ABC Fitness Solutions, the third party billing processor behind Planet Fitness memberships, and blocking a draft without a written cancellation on file can leave the account open and in arrears.
